Typically, your insurance company requires that the following questions be answered before an orthotic claim for prescription orthotics will be evaluated.  For your convenience, the GAITWAY has prepared this document for use in submitting information to insurance carriers.

Q1: Do I need a prescription from my Medical Doctor?

Yes.  Please confirm your coverage with your Insurance carrier.  In some instances, a prescription for orthotics from your Chiropractor will suffice.

 

Q2:  Was an impression of the patient’s foot taken?  If so, please state the technique that was used to create the impression.

Yes.  Positive Models made from foot casts by placing the patients foot in the neutral position of the subtalar joint in the prone position.  These will be used in the creation of the prescription orthotics.

 

Q3: Was the orthotics appliance prescribed and were the orthotics custom made?

Yes.  The GAITWAY has licensed suppliers such as a Podiatrist and Kinesiologist who are qualified to perform medical examination using a biomechanical evaluation and gait analysis.  All orthotics prescribed by the GAITWAY are medically necessary, custom made and designed for daily use.

 

Q4:  Was a biomechanical evaluation performed?  If yes, please provide the date of the examination and a copy of the biomechanical report.

Yes.  Enclosed is the Dynamic Gait and Pressure Analysis Report with the Analysis Date.

 

Q5: Who manufactured the orthotics and what are their qualifications/credentials?

The custom, prescription orthotics was manufactured by International Orthotic Labs Inc.  6777 Fairmount Dr. SE  Calgary, AB  1-800-887-7138

Licensed podiatric and medical professionals as well as other healthcare specialists staff the Clinical Services Group.  A letter from the laboratory is enclosed.

 

Q6:  What materials are used in the manufacturing of your orthotics?

The semi-rigid modules are made from a proprietary blend of fiberglass and plastics known as Duramaxx.  Posting, accommodations and top covers are made from standard podiatry materials such as crepe, dermaplast, latex, lunalastik, nickleplast/lunacell A, plastazote, poron, and vinyl.

 

Financial Program

The Gaitway professional fee is $85.  This includes a dynamic and pressure analysis report, a complete history, orthopedic physical assessments, specific muscle/injury rehabilitation, ongoing education, preventative exercises, and outstanding client care.

The orthotics are $480.00.  Your custom orthotics includes all necessary postings, custom accommodations, warranties, fitting and follow up care.

If orthotics are prescribed, upon purchase, your professional fee will be included towards the orthotic price of $425.00.  If you have any questions regarding your Coverage or have any other financial concerns, please discuss additional options with The Gaitway.

 

Warranty

Orthotic modules: warranted against cracking or splitting for 2 years.

Postings: warranted against normal wear and tear for 1 year.

Top covers and bottom reinforcements: warranted against normal wear and tear for 6 months.

 

Money Back Guarantee

The Gaitway carries a 30-day money back guarantee that commences when the initial shipment leaves our lab.  There is a non-refundable fee of $150.00

 

Modification Terms

1 – 90 days:  No charge for the first set of modifications, subsequent modifications:  $50.00.
